AUTHORIZATION AND CONSENT

I hereby authorize Health Carousel to use any photographic images and/or video recordings regarding my personal experiences and containing my personal likeness as well as any descriptions of those experiences associated with those images or recordings (collectively, the “Images”). I understand all such Image sharing is completely voluntary and not a condition of my employment with Health Carousel or any customer of Health Carousel. 

I understand that Health Carousel may use any Images for promotional, marketing, and sales efforts. These efforts may include editing the Images, creating samples from the Images, and/or posting the Images on the Company’s website or on Company-controlled social media platforms. 

I understand that the Images, and all rights associated with them, shall belong solely and exclusively to Health Carousel, which shall have the absolute right to copyright, duplicate, reproduce, alter, display, distribute, and/or publish the Images in any manner, for any purpose, and in any form, including, but not limited to, print, electronic, video and/or internet.

I voluntarily waive any and all rights with respect to the Images, including compensation, copyright, privacy rights, and any right to inspect or approve such Images and/or copy, print or other materials that may be used in connection with them.

I understand that I may withdraw this consent by emailing Health Carousel marketingsupport@healthcarousel.com my full name and a statement expressing my request to remove all or selected Images from future use by Health Carousel. To the extent possible and practical, as well as allowed under applicable law, Health Carousel will take all necessary steps to discontinue future use of the Images.
